Permanent Records Transfer

Use these forms to transfer permanent records to the State Archives

  • Permanent Paper Records:

Records Transmittal Form (ARM-14)  - used to transfer permanent paper records to the State Archives via the State Records Center staff

Non-permanent Records

  • Records Destruction 

Report on Records Destroyed Form (ARM-11) - used with general and agency-specific retention schedules for records that have met their full retention period

Digitized Records

Authorization for Disposal of Original Paper Records Stored as Digital Images (ARM-13) - used to destroy original paper records now digitized and to designate the electronic copy as the official version of the record

Microfilm

ARM-8A Microfilm Transmittal and Receipt (ARM-8A) - used to transfer security microfilm to the Archives' vault
